Become a Broker
To become an Approved Millennium Mortgage Broker, please complete and submit the Broker Application Package and appropriate documents by following the steps below:
- Download and print the Broker Application Package in PDF format.
- Complete and sign the Broker Application, Authorization to Release Information, Broker Agreement, Sanction Letter and Zero Fraud Agreement.
- Mail the signed Application Package and required documents listed in the Information Checklist to:

A Millennium Mortgage Account Manager will review your information, to confirm requirements are met, and contact you with your Broker ID number and password. If you are unable to download the Broker Application Package, please Contact Us and we will be happy to send you one.
